Measurements Form

Ok, for a custom-made cosplay item that is made especially for you, these are the required measurements. Please understand that, without these measurements, I cannot make the costume. Although I ask for your dress size, it is only to get an idea of the amount of material I will need to build your costume.

Since everyone has a different body shape, asking for only a general size would be silly. A size 14 that fits a person who is 5 foot 2 inches would be completely different than a size 14 that would fit a person who is 5 foot 6 inches.

I use these measurements to make sure the costume will fit correctly, and as a seamstress, the info you give me is always confidential. If you are too embarrassed to give out your measurements, you should not order a costume.

Also, please make sure that the measurements you give are true and correct. Never round down in sizes, thinking that you will be able to lose weight. I don't know you personally and only have the information you give me, so if you provide incorrect measurements to me and are upset that your costume doesn't fit, I cannot be blamed for that. If you have trouble doing your own measurements, please have someone help you.

All measurements are in inches.

Back of Neck to Waist  
Waist to Knee  
Knee to Ankle  
Shoulder to Wrist  
Crown of Head
Required for all headwear
Shoe Size and Width
Required for all adult footwear

If there are any additional measurements that I need, I will let you know when your order request is received, but these are the basics. This completed form must be sent with your payment. If you choose to pay by Paypal, please copy this form and email it to me. I cannot process your order without it.
